Please bring the completed forms to your initial evaluation to ensure your time is effectively utilized.  Incomplete forms will be completed in session, and will count as part of your scheduled appointment time.

Parents of clients ages 17 and below, please complete the Adult Comprehensive Assessment and Symptom Checklist as it pertains to your child.
